Um, what specific example(s) are you referring to? I mean, it's funny you say that it seems like an Ork thought it up, because well, you're right. But that doesn't make it unrealistic at all. The violence in most Warhammer 40k novels is understated when compared to a soldier in actual warfare or as a citizen/political prisoner in various totalitarian regimes during the 20th century, e.g. USSR, Communist China, East Germany, Poland, Guatemala, North Korea, etc etc. This is why I recommended you do some nonfictional reading. It might help you "get" that the characters and events portrayed in various Warhammer 40k novels (such as Gunheads, Let the Galaxy Burn, Last Chancers, etc etc) are mere shades of their 20th century counterparts.
I would really recommend that you start with reading "The Rape of Nanking" or "Khe Sahn: Siege in the Clouds." You might also want to read "Storm of Steel" (yes it's actually a nonfiction book even though it sounds like a Warhammer 40k title) or "Stuka Pilot." Both are amazing reads and all of them will make the absolute worst battle sequences in any Warhammer 40k novel pale in comparison.
