Maybe it's like a stimpack, only it's released in the smoke rather than the injection.

Or maybe marines have a new passive ability:

Second-Hand Smoke: Gives 5 damage/second to all biological units around the marine. Also has 25% chance to cause cancer.

Hey, when you're facing enemies like the Zerg, the odds of cancer being what kills you are pretty slim. Besides, sometimes the marines need to chill in between their stim shots.
